Consumer Annual Subscription Cancellation and Refund Policy – My Wellchoice+ consumer members are subscribing as an individual and/or families and thus financially responsible for their monthly or annual membership subscription. Consumer members pay a recurring subscription fee for access to the web site. Recurring Membership: Accounts are automatically renewed monthly or annually. Members will be responsible for automatic renewal charges until they notify My Wellchoice+ of cancellation or changes to current subscription plan. Cancellation or membership changes can be requested at anytime but are subject to cancellation fees. The present cancellation fee is $25 during the 1st 6 months. There is no cancellation fee after 6 months. Changes to subscription payment plan: When changes to subscription payment plans occur credits/refunds due to subscription payment plan changes will be issued prorated for any remaining months that have already been paid not including the current month. All refunds and credits apply to the membership fee only and do not include any additional services and goods purchased by a member on the web site. Cancellations: When cancellations to memberships occur, all access to the web site will be revoked on the day we receive notification of request and Reward Points/contest eligibility will be void at this time. Because payments are made in advance to prepay for service, credits/refunds due to subscription cancellation will be issued prorated for any remaining months that have already been paid not including the current month. The enrollment form needs to be sent to MWC+ by the 15th of the month for the start date of the next month. Anything after the 15th of the month will not be able to start until the following month. (Example: enrollment form received from March 1st - 15th will have a start date of April 1 – enrollment form received from March 16th – 31st will have a start date of May 1)
