Information Covered by this Policy

This Privacy Policy applies to all Personally Identifiable Information about you that is obtained by mywellchoiceplus.com, hereafter referred to as My Wellchoice+, in connection with providing you personal health improvement training informational services. Our policy will continue to cover information we may collect about you during the course of our relationship as well as after the relationship has ended. What Information We Collect and How We Use It In order to use the My Wellchoice+ website all users are required to be registered. To complete registration, you will need to provide us with your first and last name, unique identification number (for sponsored memberships only), e-mail address, mailing address, date of birth, and gender. Members entering our site directly (not through a sponsoring organization's portal) will be asked to choose a username, password, and password hint answer. All users that choose to utilize the My Wellchoice+ fitness, nutritional, or life skills programs will be prompted to provide additional personal health information. This information is required so the expert system can accurately customize a suitable program for the individual based upon health related specifications and recommendations.

How We Keep Information Secure

The privacy of your experience within our site is increased by our expert system and database having internal controls, access requirements and safeguards designed to meet our stringent privacy protocol. Access to your personal information is limited to only those employees who need to know the information to provide services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to protect personal information and we regularly review our security standards and procedures to protect against unauthorized access to personal information.

Information Disclosure

Except as set forth in this policy, My Wellchoice+ will not disclose your gathered information to any unaffiliated companies. Disclosure of information to third parties will not be used in a manner that can be extrapolated to identify or contact you ("Personally Identifiable Information") without your permission. Any information provided to third parties from My Wellchoice+ about you will be treated in accordance with this privacy policy, unless you are otherwise notified and expressly consent to such information being given. In the following limited circumstances we will consider, and may release, Personally Identifiable information to third parties without your permission: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena, or court order; or (2) in special cases such as a physical threat to yourself or others. Your Health Information Rights Access: You have the right to review or get copies of your health information, with limited exceptions. You may request that we provide copies in a format other than photocopies. We will use the format you request unless we cannot practicably do so. You may be asked to make a request in writing to obtain access to your health information. You may obtain a form to request access by using the contact information listed at the end of this Notice. We will charge you a reasonable cost-based fee for expenses such as copies and staff time. You may also request access by sending us a letter to the address at the end of this Notice setting forth the specific information to which you desire access. If you request an alternative format, provided that it is practicable for us to produce the information in such format, we will charge a cost-based fee for providing your health information in that format. If you prefer, we will prepare a summary or an explanation of your health information for a fee. Disclosure Accounting

You have the right to receive a list of instances in which we or our business associates disclosed your health information for purposes other than performing the services our site provide, for the last 6 years, but not for disclosures made prior to January 1, 2007. If you request this accounting more than once in a 12-month period, we may charge you a reasonable, cost-based fee for responding to these additional requests.

Restriction

You have the right to request that we place additional restrictions on our use or disclosure of your health information. We are not required to agree to these additional restrictions, but if we do, we will abide by our agreement (except in an emergency).

Alternative Communication

You have the right to request in writing that we communicate with you about your health information by alternative means or to alternative locations. Your request must specify the alternative means or location, and provide satisfactory explanation how payments will be handled under the alternative means or location you request.

Amendment

You have the right to request that we amend your health information. Your request must be in writing, and it must explain why the information should be amended. We may deny your request under certain circumstances. You may obtain a form to request an amendment to your health information by using the contact information listed at the end of this Notice.

Consumer Subscriptions

If you wish to purchase an individual annual or monthly membership subscription by credit card you will need to provide us with more detailed Personal Information so that we can complete your transaction. This includes a valid credit card number and expiration date, the name on the credit card, and the corresponding billing address. We employ SSL (Secured Socket Layer) during every step of the registration process. Your credit card information will be encrypted when stored. If you wish to purchase an individual annual or monthly membership subscription by bank draft then you will need to provide us with more detailed Personal Information so that we can complete your transaction. This information includes, but may not be limited to, primary account holders name, bank name, bank routing number, bank account number, bank and personal addresses.

Cookie Technology

My Wellchoice+ employs cookie technology to allow users to move more quickly through our site. In general, a cookie is small piece of data stored on the user's computer tied to information about the user. My Wellchoice+ has chosen to use session ID cookies for increased security reasons. The user's session ID cookie stores only an identification ID that the My Wellchoice+ system uses to identify a user as they move throughout the site. The session ID cookie itself does not contain any personal identifying information. With session ID cookies, once a user closes the browser, logs out, or goes for extended periods of time without activity, the cookie simply terminates.

You are always welcome to contact us in order to delete your personally identifiable information from our databases, to update your profile, or request a record of the personal information view history. We will continually evaluate our privacy policy to ensure that your anonymity is maintained and to protect your personal information.
